Integration, collaboration and service sustainability - weighting reflective the aims of the service provision to support integration and collaboration with wider health and care system. \nKey Criteria 4. Improving Access, reducing health inequalities and facilitating - weighting reflected the nature of the service provision linked a specific cohort of the population.\nKey Criteria 5.
