The service is broken down into five core elements.\u003cbr/\u003e24/7 Rapid Response Service - Led by nurses and offering urgent specialist care and support, including symptom management and non-medical prescribing, during both regular and after hours. It is designed for patients considered to be in the last eight weeks of their lives who wish to receive care in their own homes.\u003cbr/\u003eLocal Coordination Service - A 24/7 service providing information, advice, signposting to services and face-to-face interventions for patients, caregivers, family, and friends.\u003cbr/\u003eHospice at Home \u2013 Offering a more integrated approach for patient assisting those who wish to spend their last days at home. This includes rapidly discharging patients from hospices and hospitals to avoid unnecessary admissions.\u003cbr/\u003eDay Care Service \u2013 Aimed at patients who are in the last eight weeks of life and prefer to receive care at home. Short episodes of care are delivered throughout the day as needed to ensure patients\u0027 needs are met. \u003cbr/\u003eNight Care Service - Designed for patients who are in the last eight weeks of life and prefer to receive care at home. This service will ensure that the patient\u0027s needs are met through short episodes of care provided during the night when appropriate.\u003cbr/\u003ePlease note, this Contract Notice relates to a service/contract in scope of the Provider Selection Regime (PSR). The Procurement / Provider Selection Process conducted will be subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Procurement Act Regulations 2023 will not apply.\u003cbr/\u003e\u003cbr/\u003eThe contract will be for a period of 5 years with the option to extend for up to a further 3 years, commencing 1st April 2026.\u003cbr/\u003e\u003cbr/\u003eThe maximum annual contract value is \u00a31,930,000 exclusive of VAT.\u003cbr/\u003e\u003cbr/\u003ePlease note there is a financial threshold associated with this procurement.
