The service delivers one-to-one Specialist Palliative Care to patients in their normal place of residence. The service is provided for those in whom diagnosis has shown that the advent of death is likely and not too distant and for whom treatment includes a large element of palliation or has been identified as being in the last days of life.  \nThe service is delivered 365 days per year, for night care. Night visits are 9 hours long and delivered between the hours of 2200 and 0700. \nThis Service is for the Tameside Locality within NHS Greater Manchester Integrated Care Board (NHS GM ICB).\nThis contract will be for 12-months from 1st April 2025 - 31st March 2026.\nThe total value of this contract will be \u00a349,946 plus Nationally mandated tariff uplift. \nThis is a Provider Selection Regime (PSR) Direct Award Process C intention to award notice, this notice serves as an intention to award under the PSR.",
