The heart failure component focuses on medication optimisation, patient education, and end-of-life planning, with strong MDT collaboration and integration with rehabilitation and hospice services.   \u003cbr/\u003e \u003cbr/\u003eIntegrated Community Respiratory Service: A specialist-led service for diagnosing and managing respiratory conditions such as COPD and asthma. It includes spirometry, home oxygen assessments, clinics, and ANP-led reviews.
