Evidence of current service delivery against the PSR Key Criteria and the breakdown of Key Criteria weighting in relation to this service for the purposes of awarding a contract under Direct Award C is detailed below:  \nIntegration, collaboration and service sustainability 30%\n\u2022\tThis service provides multi-disciplinary input and support,critical to the recovery and experience of the patient. \n\u2022\tThis provider forms part of a cohesive system partnership between providers, allowing wide opportunities for stroke survivors to reach their rehabilitation potential.\n\u2022\tThe provider utilises existing community assets, transport services and community and voluntary services.\n\u2022\tThrough regular meetings with partners the service reduces duplication and supports the sustainability of local services. \n\u2022\tThis provider contributes at the weekly Medically Fit For Discharge meeting.\nQuality and innovation 20%\n\u2022\tQEF has consistently maintained a service of the highest quality for stroke survivors, meeting the ICBs requirement for quality and safety.\n\u2022\tPatients receive a structured, personalised and seamless service that enables them to feel supported through their rehabilitation. \n\u2022\tThis provider continually fulfils all quality requirements.\n\u2022\tThis provider is an integral member of the Local Stroke Working Group.\n\u2022\tProvider delivers the Stroke Inpatient Rehabilitation specification\nValue 20%\n\u2022\tThe provider ensures an ability to utilise funding to maximise benefits to stroke survivors.\n\u2022\tThe service ensures individualised needs and preferences are met where it will benefit stroke survivors most, while also offering excellent value for money and delivering on outcomes.\n\u2022\tA tailored service adds value to individuals and the system, providing a crucial prevention aspect of care to support whole system efficiency around the use of resources.\n\u2022\tFunds will continue to be utilised efficiently and effectively to deliver a sustainable, efficient, and cost-effective service.\nImproving access, reducing health inequalities and facilitating choice 20%\n\u2022\tThe provider demonstrates all provision of service is accessible to all patients who are eligible for the service. \n\u2022\tThe service is available to all patients registered to an East Surrey GP who have experienced a stroke and whose rehabilitation meets the criteria for admission. Referral and admission to QEF is made in consultation with patients and family/carers ensuring all rehabilitation options in the community are explored to ensure the right rehabilitation outcome.\n\u2022\tQEF work with acute hospitals to ensure a timely referral and admission from stroke units to QEF rehabilitation.\n\u2022\tQEF work closely with the community Early Supported Discharge team to enable timely discharge and step down from inpatient rehabilitation to meet onward rehabilitation needs. \nSocial value 10%\n\u2022\tQEF is an active organisation in the local community through its charity status\n\u2022\tThe organisation provides local recruitment and training opportunities. \nThis is a Provider Selection Regime (PSR) contract award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023.
