    "description": "NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission Support at Home (Domiciliary Care) Services - North East Lincolnshire through an Open procurement process.\r\nThis procurement seeks to ensure the delivery of four distinct service lots to meet the needs of adults requiring Support at Home (Domiciliary Care) across North East Lincolnshire (NEL).\r\nThe Contracting Authority wishes to establish three (3) Providers for Lot 1 consisting of three (3) Geographical Lead \u0027Locality Providers\u0027; and one (1) Lead Provider per each of Lots 2-4 consisting of: one (1) Lead Provider for Enhanced Support at Home; one (1) Lead Provider for Carers Breaks; and one (1) Lead Provider for System Resilience.\r\n1. Geographical Areas (Lot 1 (a, b, and c))\r\n\u2022 Delivery of high-quality domiciliary care services to individuals living within their specified geographical area.\r\n\u2022 Implementation and development of the \u0027Teams not Times\u0027 model that improves responsiveness, flexibility and person-centred support.\r\n\u2022 Includes management of existing extra care housing (2 schemes) (where applicable) to support independent living.\r\n\u2022 Services must focus on maintaining independence, reducing risk, improving the quality of life, and maintaining people in their homes for longer.\r\n2. Enhanced Support at Home (Lot 2)\r\n\u2022 Provision of intensive support to individuals with complex care needs.\r\n\u2022 Includes promoting health resilience and avoiding unnecessary hospital admissions.\r\n3. Carers Breaks Provision (Lot 3)\r\n\u2022 Delivery of flexible respite care to support informal carers.\r\n\u2022 Must include planned and emergency breaks with tailored services to meet carers\u0027 and service users\u0027 needs.\r\n4. System Resilience Provision (Lot 4)\r\n\u2022 Support during peak times and emergencies.\r\n\u2022 Services include short-term interventions to stabilise individuals and facilitate timely discharges from acute settings.",
