    "description": "NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission Support at Home (Domiciliary Care) Services - North East Lincolnshire through an Open procurement process.\r\n\r\nThe four Lots to be procured are:\r\n1.\tGeographical areas\r\na.\tGeographical area West (plus one Extra Care Housing scheme within this area)\r\nb.\tGeographical area East\r\nc.\tGeographical area South (plus one Extra Care Housing scheme within this area)\r\n2.\tEnhanced Support at Home Provision\r\n3.\tCarers Breaks Provision\r\n4.\tSystem Resilience Provision\r\n\r\nThe preferred Bidders will be required to demonstrate they are able to provide a high-quality, culturally sensitive Support at Home (Domiciliary Care) services that meets CQC regulatory requirements, and the principles as defined by the Care Act 2014. Bidders will demonstrate achievement of individualised outcomes as defined by the care/support plan and will demonstrate an ability to maximise potential in relation to physical, intellectual, emotional and social capacity, to support people to remain in their homes for longer. Where Bidders indicate a wish to be considered for the carers breaks provision, Bidders will demonstrate a supportive approach to enable informal carers to sustain their caring roles, enabling the reduction of carer burden.\r\n\r\nThe Contracting Authority wishes to establish three (3) Providers for Lot 1 consisting of three (3) Geographical Lead \u0027Locality Providers\u0027; and one (1) Provider per each of Lots 2-4 consisting of: one (1) Lead Provider for Enhanced Support at Home; one (1) Lead Provider for Carers Breaks; and one (1) Lead Provider for System Resilience.  \r\n\r\n1.\tGeographical Areas (Lot 1 (a, b, and c))\r\n\u2022\tDelivery of high-quality domiciliary care services to individuals living within their specified geographical area.\r\n\u2022\tImplementation and development of the \u0027Teams not Times\u0027 model that improves responsiveness, flexibility and person-centred support.\r\n\u2022\tIncludes management of existing extra care housing (2 schemes) (where applicable) to support independent living.\r\n\u2022\tServices must focus on maintaining independence, reducing risk, improving the quality of life, and maintaining people in their homes for longer.\r\n\r\n2.\tEnhanced Support at Home (Lot 2)\r\n\u2022\tProvision of intensive support to individuals with complex care needs.\r\n\u2022\tIncludes promoting health resilience and avoiding unnecessary hospital admissions.\r\n\r\n3.\tCarers Breaks Provision (Lot 3)\r\n\u2022\tDelivery of flexible respite care to support informal carers.\r\n\u2022\tMust include planned and emergency breaks with tailored services to meet carers\u0027 and service users\u0027 needs.\r\n\r\n4.\tSystem Resilience Provision (Lot 4)\r\n\u2022\tSupport during peak times and emergencies.\r\n\u2022\tServices include short-term interventions to stabilise individuals and facilitate timely discharges from acute settings.\r\n\r\nAll Bids must be submitted through the Atamis e-procurement portal - https://atamis-1928.my.site.com/login\r\n\r\nAdditional information: e-Procurement portal - https://atamis-1928.my.site.com/s/Welcome\r\n\r\nBidders can access the opportunity and procurement documentation through the Atamis e-sourcing portal -\r\nhttps://atamis-1928.my.site.com/s/Welcome\r\n\r\nContract reference C337775\r\nThe closing date for tender responses is 13:00 on 19 March 2025.\r\n\r\nNHS Humber and North Yorkshire Integrated Care Board (ICB) is acting as the Contracting Authority (and Co-ordinating Commissioner) for the Support at Home (Domiciliary Care) North East Lincolnshire.",
