Framework for the provision of Keeping Well Services Services that are being commissioned under this Framework are: Lots 1 to 5 - Care & Support at Home (Daytime) • Lot 1 - Sunderland East • Lot 2 - Sunderland West • Lot 3 - Coalfields • Lot 4 - Washington • Lot 5 - Sunderland North Lot 6 - Care and Support in the Home: Overnight Care Lot 7 - Live in Care Service Lot 8 - Carer Support Service (Carer Relief) Lot 9 - Support Services (Ad-Hoc) Care and Support in the Home The framework will be awarded as follows: Lots 1 to 5 - single contractor for each geographical locality area lot as stated above. All framework orders will be directly placed with the contracted provider. Lot 6 - single contractor lot covering all locality areas citywide. All framework orders will be directly placed with contracted provider. Lot 7 - single contractor lot covering all locality areas citywide. All framework orders will be directly placed with contracted provider. Lot 8 - single contractor lot covering all locality areas citywide.
