They will also be supported through targeted individual or group activities which not only focus on them but also take a \u00e2\u0080\u0098whole family\u00e2\u0080\u0099 approach working with other agencies, including Health Visitors, School Nurses, Schools, NESCOL, Universities, Targeted Family Support and Fit Like Family Wellbeing as well as services working directly with the cared-for person.\r\n There is an interdependency between services and providers will be expected to work collaboratively to support for those transitioning from Young Carers to Adult Carers services ensuring they have support from whole system.\r\n The contracts will commence on 1 April 2025 and will operate for five years until 31 March 2030 plus the option for a two year extension.\r\n The maximum funding for Year 1 of the contract will be:\r\n Young Carers Service - GBP206000\r\n Funding for future years will be subject to any agreed sectoral uplifts..
